A meeting with Mexican mentors in Mexico City marked the conclusion of the successful training programme that seeks to improve the implementation and management of the Multidisciplinary Orthogeriatrics Functional Units (UFMO).

The IOF OrtogeriatrIMSS Mentor Training Programme is a pioneering initiative of the Medical Innovation Directorate of the Mexican Social Security Institute (IMSS) in collaboration with the International Osteoporosis Foundation (IOF), to address a serious and growing problem: hip fractures in the elderly.

The figures are alarming. One in 10 people who have a hip fracture die within the first month, and over the next year, up to 25 % more may lose their lives due to complications and reduced mobility. In Mexico, more than 8,000 hip fractures were reported in 2019 within the Mexican Social Security Institute (IMSS) alone, representing an increase of 42 % in women and 40 % in men compared to 2006.

It is estimated that IMSS performs around 60 per cent of all hip fracture surgeries in Mexico. Since 2022, the National Strategy "OrtoGeriatrIMSS" is focused on implementing comprehensive and interdisciplinary management of hip fracture in people aged 65 and older in second and third-level care units, to reduce hospital stay, complications, disability, mortality and hospital costs related to fracture. What is the strategy? During hospitalisation, patients are cared for by a multidisciplinary team that makes up the Functional Multidisciplinary Orthogeriatric Unit (UFMO) led by a coordinating team comprising a geriatrician and an orthopaedist and composed of rehabilitation physicians and other health professionals, including nurses, nutritionists, social workers and psychologists. This integrated approach not only enhances medical care, but also involves caregivers by providing information on mobilisation, diet, and mental health of patients.

To foster the implementation and management of UMFOs according to best practice standards, the International Osteoporosis Foundation (IOF) and IMSS have collaborated on the IMSS OrtogeriatrIMSS Mentor Training. This collaboration aims to create a community of mentors trained according to the internationally recognized Best Practice Framework Standards established through IOF's Capture The Fracture® programme, with a focus on secondary fracture prevention. 

The training programme was conducted in a hybrid format, combining virtual and in-person sessions, with a face-to-face closing day on January 26th in Mexico City. The six virtual sessions ran weekly from November 2023 to January 2024. Dr Juan Humberto Medina Chávez, head of the Division of Clinical Excellence at the Coordination of Innovation in Health, emphasises the importance of this programme: "By training future mentors, we ensure that they have the knowledge, skills, and competencies necessary to support the Multidisciplinary Functional Units of Orthogeriatrics (UFMO) in their goal of providing high-quality care. In addition, it fosters the development of experienced teams to train orthogeriatric teams in second and third-level IMSS hospitals in specific functions."

Dr. Philippe Halbout, CEO of the IOF, added: "The OrtoGeriatrIMSS mentor training programme marks a significant milestone in improving the quality of care provided to people with fractures, underpinned by the best practices of the global Capture The Fracture® programme.
